Amazing Smiles Membership Plan
We know that dental care costs can add up, especially without dental insurance. The Amazing Smiles Membership Plan is designed to help you save on the care you need. For one low annual or monthly payment, you get access to preventative care, including exams, X-rays, and cleanings, as well as discounts on restorative and cosmetic services. There are no deductibles and no annual maximums.
*This is not an insurance policy. This is a discount dental plan valid ONLY at Amazing Smiles Family Dental Care. Any service not paid at the time of service will be billed at our usual fees and is subject to a billing charge of 1.5% per month unless other arrangements are made. Annual charges are non-refundable and cannot be combined with insurance plans. Plan fees are valid only. All plan fees and treatment fees, as listed, are subject to change without notice.

Adult Plan
$500 (paid in full), $540/Year OR $45/Month (with $135 down)
- Two professional exams per year
- Two cleanings per year (does not include periodontal cleanings)
- Two fluoride treatments per year
- Four bitewing and six posterior-anterior X-rays (panoramic/FMX as needed)
- One CT scan
- Unlimited emergency visits with necessary X-rays and exams
- 20% off most dental services
Child Plan
Ages 15 and Under
$483 (paid in full), $504/Year OR $42/Month (with $126 down)
- Two professional exams per year
- Two cleanings per year (does not include periodontal cleanings)
- Two fluoride treatments per year

- One CT scan
- Unlimited emergency visits with necessary X-rays and exams
